What this notice covers

This notice covers the public Alleviate Labs pages and Learn More form. It does not replace notices or terms for the patient app or Care Portal.

Collection

Information we collect

The Learn More form collects your name, email address, audience type, area of interest, and your agreement to let us respond. You may also provide an organization and message. When available, the site records referral and campaign parameters, along with the submission time and request status.

Do not submit symptoms, diagnoses, medication information, treatment details, private health information, or other sensitive personal information.

Use

How the information is used

Alleviate Labs stores submissions in the project database to review and respond to questions, understand who is interested, and record consent and request status. This release does not send automated marketing or transactional email or connect submissions to a customer relationship management (CRM) service.

Retention

Storage, access, and retention

Retention period not yet set

No public data-access service lets people list or read submissions.

We have not set a fixed retention period. Records stay stored until an authorized operator reviews or deletes them.

No internet transmission or storage can be guaranteed to be completely secure.

Use the Learn More form

Agreement is required to submit the form. To request access, correction, or deletion of a submission, use the Learn More form and write “Privacy request” in the interest field. Include only the contact information needed to identify the submission; do not include health details.
